Vendors are welcome to submit a booth request form for approval by filling out the request below to submit online. OR completed requests can be emailed to firstname.lastname@example.org booth spaces are assigned on a first come first served basis.
Please complete the entire form before submitting. (You may bring it with you the day of the fair if you are willing to take whatever space is available at that time if you are a known Vendor the church as worked with before.)
Church grounds open at 8am on Sat May 12 and close at 1pm. You may unload at your booth site until 8:45am, Pet Fair opens to the public at 9am and runs to noon. You may not park your vehicle at your booth or in the fair area after 8:45am or before noon.
The Medina Valley United Methodist Church Inc., shall not be liable for any loss, or damage of property, or injury of vendors, staff or family. The Vendor and their staff, employees or family promises and agrees to make no claim for any damages, loss or injury.
